Consent forms & minors
Everyone getting tattooed fills out our consent form (we’ll have copies in the shop, or print yours ahead to save time).
Minors must be accompanied at all times by a legal guardian and bring proof of guardianship (I.E.: I.D., birth certificate, passport, etc.). Both the minor and guardian need valid photo ID, and the guardian stays for the entire procedure.
